Pennsylvania measles outbreak tops 1,000 cases after fifth associated death

The state has recorded 1,004 confirmed infections across 39 counties. Nearly 200 people have been hospitalised during the outbreak.

Pennsylvania health officials reported on September 30 that an unvaccinated Lancaster County resident who tested positive for measles had died, becoming the fifth person classified as a measles-associated death in the state’s 2026 outbreak. By October 5, Pennsylvania had recorded 1,004 confirmed cases across 39 counties.45

Cases pass a significant threshold

The outbreak, which health officials said began west of Philadelphia in April and was linked to low vaccination rates, is Pennsylvania’s largest in more than three decades. At least 198 people have been hospitalised, while Lancaster County remains the most affected area, with 391 cases reported.125

The total increased by 106 cases in the week before October 6. Earlier state figures, dated September 28, listed 943 cases, 176 hospitalisations and five measles-associated deaths. The later totals show that infections continued to rise while the reported death count remained unchanged.47

The state’s health department said the fifth person to die was an unvaccinated Lancaster County resident who had tested positive for measles. By October 2, Pennsylvania’s five reported deaths included two infants and three adults or older adolescents.47

Vaccination response

Deutsche Welle reported that almost every recorded case in Pennsylvania was among unvaccinated people. The state health department said only four cases were recorded among vaccinated people, representing less than 1 per cent of its earlier total. It says two MMR doses provide 97 per cent lifetime protection.24

Staff at state health centres have given over 5,500 MMR doses at 180 pop-up clinics serving affected areas. Department nurses had delivered more than 8,500 doses during 2026 by the September update, while providers across Pennsylvania administered more than 49,000 doses in August and more than 53,000 in September by the time of that report.4

According to the health department, measles may still be transmissible through the air or from surfaces for up to two hours after an infected person departs. Symptoms may appear seven to 21 days after exposure, and the department describes full vaccination as the strongest protection.4

Concern beyond Lancaster County

Public-health officials are preparing for infections to spread beyond the main concentration in Lancaster County. Jeanne Franklin said Chester County, which had the state’s third-highest total at 83 cases, was preparing for more infections arriving from Lancaster and warned that transmission could reach people outside Plain Amish communities.1

Many Amish residents have not been vaccinated, but Franklin said their decisions should not simply be characterised as opposition to vaccines. She said some residents had encountered false claims that vaccination causes sterility or involves a microchip, while others were seeking immunisation after seeing neighbours affected.1

Local capacity varies across Pennsylvania: only seven of its 67 counties have their own health department. According to The Guardian, creating one requires a referendum but is not mandatory. Montgomery County is working to provide vaccination regardless of insurance status, while its medical director, Richard Lorraine, said immunisation was a central focus because measles was present around the county.1

A wider US resurgence

The United States is experiencing its worst measles year since 1991. The Centers for Disease Control and Prevention recorded 3,887 confirmed cases as of October 1, while outbreaks were also reported in South Carolina, Texas and Utah. By October 3, New York had logged 108 cases, prompting Governor Kathy Hochul to declare a state disaster emergency for one month.12

The Guardian reported that Pennsylvania’s cases were excluded from the CDC count because of a continuing disagreement over how measles-related deaths were classified. Health secretary Robert F Kennedy Jr questioned whether Pennsylvania’s reported fatalities were directly caused by measles, while Governor Josh Shapiro called Kennedy incompetent.1

The outbreak’s complete scale remains uncertain. Paul Offit, who heads the Vaccine Education Center at the Children’s Hospital of Philadelphia, said: “I think we don’t know how bad.” International health officials will meet in November to assess whether the United States and Mexico no longer qualify as measles-free.13

Why it matters

For European readers, Pennsylvania forms part of the worst US measles year since 1991, with outbreaks reported across several states and national vaccination coverage below the 95 per cent level described as necessary to prevent outbreaks. International health officials are due to review the measles-free status of the United States and Mexico in November.12
